If you’d like to become a patient, simply request a New Patient Packet by calling our office at 203-861-7810 or email to us as Office@Mendribil.com.
Before we set up your initial consultation appointment, we ask you to complete a few forms about about your symptoms, health concerns and goals. Please mail or fax these to Dr. Mendribil’s office (fax 203-567-8444) at least 48 hours prior to your first visit so Dr. Mendribil will be able to review and prepare your case.
Once we receive your documents, we will contact you to set up an appointment for your initial 1 hour consultation with Dr. Mendribil.
During the initial consultation Dr. Mendribil will listen carefully to you and your concerns. He will discuss your history and health goals, and determine the best approach for your unique needs. He will also conduct an exam and order any necessary laboratory tests at that time. Some tests are performed in the office, some take home kits, and for others you’ll need to visit a convenient blood draw facility.
At the next visit Dr. Mendribil will review the findings from any lab tests and develop a personalized treatment plan for you. This treatment plan will address both your immediate symptoms and long term health goals.
As you start following the program outlined by Dr. Mendribil, you will be required to have follow-up appointments every 3-12 weeks, depending on your unique condition. Follow-up testing may also be required. This is for your good and to ensure that we get the best results for you and your health.
Insurance and Payment Options
Payment is due in full at each visit. We accept cash, check, Visa, MC, an AMEX. We DO NOT accept insurance assignment, however we do provide you with an insurance billing form to submit to your health insurance provider for reimbursement. There may be additional charges for laboratory tests and/or nutritional supplements. Some of the laboratories will bill your insurance on your behalf. Others require payment before they will process your samples.
